Post by Jobing - Our pharmacists are responsible, as per protocol, for antibiotic dosing, TPN dosing, and anticoagulation dosing and adjustments. Some of the other primary responsibilities include:Preparing and dispensing medications (as per physician orders) according to established policies, procedures, and protocols,Reviewing each medication order for safety and appropriateness and communicate with the prescribers when indicated,Ensuring safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols,Utilizing concepts of age/developmental stages in interactions with patients and families. Southwest Idaho Advanced Care Hospital is a 40â€"bed, long term acute care hospital. We are the newest member of the southern Idaho and eastern Oregon health care continuum. At SIACH, we believe your recovery means more than just physical. At our new stateâ€"ofâ€"theâ€"art facility, we offer longâ€"term acute care and critical care services for patients recovering from serious illnesses or injuries.Our patients will typically have a medically complex condition that requires more intensive services than found in other postâ€"acute settings, such as skilled nursing facilities. The physicians that comprise our team are knowledgeable of the acute care setting of a traditional hospital and, therefore, possess the clinical expertise to treat a variety of medical conditions including: â€"Stroke â€"Trauma â€"Cardiovascular Disease â€"Ventilator Weaning â€"Wound Healing â€"Respiratory Failure â€"Amputation â€"Infectious Diseases Minimum REQUIRED Qualifications: • PharmD or Bachelor’s Degree in Pharmacy with current Idaho license and Controlled Substance license • Excellent written and verbal communication skills • Ability to work effectively as part of a multidisciplinary team • Competency in basic pharmacokinetic dosing of antiobiotics, antibiotic stewardship, renal dosing, IV to PO dosing, and anticoagulant managmenet • Ability to supervise hospital pharmacy technicians in workflow and regulatory compliancePreferred Qualifications: • Previous hospital experience or residency
