Background and-or Security Investigation required DUTIES:
The Clinical Pharmacy Specialist (Telehealth ) is responsible for coordination and provision of clinical pharmacy services to patients recei! ving their care through the Clinical Telehealth modality. The Clinical Pharmacist Specialist applies clinical knowledge and skills when performing pharmaceutical functions and patient care activities and serves as a provider with prescribing privileges as outlined in the individual's Scope of Practice for Clinical Pharmacy Specialist (CPS). The primary responsibility of the Clinical Pharmacy Specialist is the provision of evidence-based pharmaceutical care services through both interaction with patients and providers. The Clinical Pharmacy Specialist performs but is not limited to the following duties:

  • Obtains medication histories from patients and summarizes significant findings in the medical record and/or to the provider.
  • Routinely reviews patients' medical records and interviews patients to detect drug-related problems.
  • Monitors laboratory results for therapeutic response and side effects.
  • Assists patients on the proper use a! nd care of their medications.
  • Maintains a current knowledge of! therapeutics and disease management.
  • Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues.
  • Designs, implements, and monitors therapeutic drug plans through direct interactions with patients and providers.
  • Assesses the stability of chronic disease states as it relates to drug therapy.
