Description:
We are looking for self-motivated pharmacy technicians who are excited to work for an innovative start-up company to implement pharmacy best-practices and improve health outcomes. Our Pharmacy Care Advocates are the face of the company, working in healthcare settings and patient homes to provide medication management services. We use modern day technologies to conduct face-to-face patient visits between the Medication Management Team and our patients. The Pharmacy Care Advocate will meet with patients in the community, conduct health screenings, assess the patient surroundings, and document the care plan. Pharmacy Care Advocates must also be able to understand and navigate health insurance formularies & pharmacy benefits for patients to help them get the most out of their coverage.
Work can be on a contract basis or part-time, with the potential for becoming full-time.
Duties:
- Meet with patients to conduct health screenings and assess their physical surroundings
- Utilize video technology to complete a face-to-face visit between the patient and a Converge Rx pharmacist
- Document the patients care plan in electronic health records
- Complete follow up phone calls with patients to ensure they are adhering to their care plan
- Work with health insurance companies to navigate the prior authorization process and negotiate medication synchronization opportunities
- Learn about & educate patients on their formulary & pharmacy benefit details
Required Skills:
- Friendly & outgoing with great communication skills
- Excellent typing & documentation skills
- Basic knowledge of prescription drug names and disease states
- Great attention to detail
- Professional demeanor and ability to represent company standards with customers
- Excellent driving record with an ability to independently navigate between patient visits and healthcare settings
- Ability to easily multi-task between different computer systems
- Technologically adept with a proven ability to utilize and problem solve varying software applicati ons
- Proficiency in Microsoft Word and Excel
