ESSENTIAL JOB QUALIFICATIONS/PREREQUISITES:
I. Must be a graduate of an accredited school of pharmacy with a minimum of a BS degree.

II. Must be licensed by the Idaho State Board of Pharmacy.

III. A minimum of 3 years in institutional pharmacy practice.

IV. A minimum of 2 years supervisory experience preferred.

Job Summary:
A. Essential Job Functions:
Able to perform all essential job duties as defined by Pharmacist Job Description

1. To hire, train! , maintain, and motivate adequate staff of registered pharmaci! sts and ancillary personnel to serve the needs of the patients and the Medical Center staff.

2. To develop and implement a medication distribution system which will fulfill the requirements of the Medical Center.

3. Formulate, compile, and update the pharmacy policy and procedure manual in accordance with JCAHO and the American Society of Health-System Pharmacists recommendations.

4. To compile and update the hospitals drug formulary with additions and deletions which must be approved by the Pharmacy and Therapeutics Committee.

5. To serve on committees as assigned by administration to include, but not limited to, P&T, Patient Care Operations, and Operations Council.
