Heritage Health is North Idaho’s premier provider of integrated medical, dental and behavioral health services. The Idaho State University College of Pharmacy and Heritage Health are jointly establishing primary care pharmacotherapy services at a busy community health center located in Coeur d’Alene, Idaho. Services will include medication adherence support, disease state management, and medication therapy management for primary care patients in an ambulatory care setting. We are seeking a clinical pharmacist faculty members to work with the primary care team and to establish clinical rotation sites for 4 th year Doctor of Pharmacy students.
Specific duties: 1) Provide clinical pharmacy services for patients with diabetes, hyperlipidemia, hypertension, asthma, and other chronic medical conditions upon consultation with providers to improve patient outcomes and patient safety; 2) Review patients’ records for efficient dru! g therapy, drug interactions, adverse drug reactions, contrain! dications, and allergies; adherence assessment and follow through with resolving medication therapy issues; 3) Precept fourth-year pharmacy student completing rotations in ambulatory care; 4) Develop a program of scholarship in primary care pharmacotherapy; 5) Provide service to the clinical site, university, and community.
